Review information to determine if an individual may have committed an IPV. If IPV is suspected, refer the case to the Office of Quality Assurance Client Fraud Investigations following the Suspected IPV Process.  Notify the Office of Quality Assurance Client Fraud Investigations of any changes in the EDG’s situation.

Identifying the Accused Individual

The individual who intentionally committed the SIPV is the accused individual. The accused individual can be any of the following:

Send the following information to the Regional Office Client Fraud Investigations Unit covering the city/town of the individual’s home address:

  • Completed W-109CF for individual’s not converted to ImpaCT.
  • Completed electronic referral through the ImpaCT Referrals module for individuals converted to ImpaCT.
  • All related information and records.
  • A description of the EDG’s current status and the accused individual, including identification of potential individual misstatements as defined in the Client Fraud Investigations referrals protocol.
